Appointments & Other Details

For your convenience, we book appointments for Monday - Friday between 7 am and 7 pm. Often you will be seen several times during a week, on the same days and time for consistency.

Appointments:
Initial appointments may be made by phone or in person. Appointment hours are 7 a.m. to 7 p.m., Monday through Friday. Please be prepared to provide your insurance information, doctor's name, and a brief diagnosis or description of your problem. Your initial evaluation will likely be 60 minutes, occasionally longer. Follow-up visits are 45 minutes to one hour.

Insurance Details:
We are participating providers for all major insurances. Our office can contact your insurance company to determine the number of visits you are allowed.

We are required by law to collect co-payments for visits at the time of service. You may pay by cash or personal check. We are not able to process debit or credit cards at this time.

What to Bring:
Please bring your doctor's prescription/request for physical therapy, your insurance card and co-payment, along with any operative reports, x-rays or MRI's if available. You will also want to bring or wear comfortable clothes and/or sneakers so that you can participate in physical therapy. For upper extremity care (shoulders, arms, neck, wrists) you will want to have a short sleeved t-shirt. For lower extremity care (hip, knee, ankle, foot) or back you will want to have shorts or sweatpants and sneakers.

To Cancel or Reschedule an Appointment:
A fee will be charged for any appointment missed or rescheduled with less than 24 hours notice. Please contact the office as soon as possible so that we may reschedule you promptly.
